About the Procedure

Dr. Fechner routinely performs our eyelid lift in Worcester as an outpatient procedure in the office. Usually, relaxing medication is administered to help the patient feel relaxed and comfortable. Patients routinely sleep during the majority of the procedure. Deeper sedation may be used, but typically only when an eyelid lift is combined with other facial cosmetic procedures. Dr. Fechner prefers to avoid general anesthesia.

Blepharoplasty with An Internal Brow lift

In some patients, a brow position that is too low contributes to the overall droopy appearance of the eyelids. In addition to the various brow lift techniques, Dr. Fechner performs a brow-release that can be achieved while performing the blepharoplasty. Dr. Fechner will determine a patient’s candidacy for this advanced internal brow lift during the personal consultation.

Eyelid Surgery Steps

Initially, Dr. Fechner will carefully mark the location of the planned incisions and precisely measure the amount of skin removal. To maximize cosmetic healing and avoid visible scars, Dr. Fechner places the incisions in the most inconspicuous location. After the eyelid skin has been numbed, skin, muscle and fat will be removed, lifted and repositioned for the most pleasing and natural eyelid appearance. Using delicately performed surgical techniques, any bleeding is minimal. To assist further, tiny capillaries are sealed to limit bruising and swelling. All incisions are then carefully closed with tiny, mostly absorbable sutures.

Excess fat in the lower eyelid may be removed through a small incision beneath the eyelashes. Most commonly, Dr. Fechner will reposition this fat into the tear trough region to achieve a smooth transition from cheeks to lower lids. In addition, he removes a tiny amount of excess skin to produce a smoother, less wrinkled appearance. Most patients benefit from a conservative reduction of excess skin in this region. Alternatively, the incision can be placed through the inside lining of the eye in rare cases where extra skin does not affect appearance. A blepharoplasty procedure takes from one to two hours, depending upon the extent of the areas to be treated.

Advanced Techniques

In some patients, Dr. Fechner applies advanced blepharoplasty techniques, where the lower eyelid fat is not removed but repositioned to fill the under-eye depression that is responsible for a hollow appearance and unsightly dark eye circles. Dr. Fechner will speak with you about the most effective method to improve your appearance, and whether a fat repositioning technique is appropriate in your case.

Lower Eyelid Canthopexy And Canthoplasty

These procedures may be performed in conjunction with a traditional lower eyelid lift. Canthoplastic techniques are useful to maintain support of the lower eyelids to avoid the sagging associated with aging skin and tissue. If your lower eyelid droops, revealing the white of the eyes more than normal, or gives you an unattractive appearance, a canthoplasty may be used to correct this condition. It is usually reserved for patients with excessive laxity of the lower eyelid.

Eyelid Skin Resurfacing

Eyelid resurfacing is a procedure designed to improve the skin appearance around the eyes. It may be helpful to improve fine lines and wrinkles as well as sun and age spots.

Dr. Fechner commonly performes laser resurfacing or chemical peels for the skin around the eyes. He will discuss the options for a resurfacing procedure and whether this treatment will enhance the results of your eyelid lift procedure.

Risks of Blepharoplasty

Complications from cosmetic eyelid surgery are rare, and most patients are exceptionally happy with the outcome. Virtually all patients develop some degree of swelling and bruising. Possible but unlikely complications include: bleeding, delayed healing, infection, drooping of the upper or lower eyelid, asymmetry, double vision, dryness or irritation of the eyes and the potential for the need for further procedures.

Recovery After Blepharoplasty

At completion of the procedure, cold compresses will be applied to your eyelids. You will continue these measures for the first few days to help minimize bruising and swelling. The area surrounding your eyes may appear puffy and slightly discolored for a few days following the surgery. You should keep your head elevated above the level of the heart in the early stages of healing. There will be minimal or no pain after eyelid surgery, although you will be prescribed pain medication to use if needed. Occasional eye irritation is controlled with the use of eye drops and gels. Initially, your eyes may fatigue more easily.

Most of the stitches used will dissolve over the first seven days. Remaining sutures are painlessly removed at this point. Average swelling and bruising may persist for up to ten days. Most women patients just wear makeup to conceal any remaining discoloration. Most patients can return to work and social activities within a week to ten days. Dr. Fechner recommends postponing strenuous exercise for fourteen days after the eyelid procedure.

Appearance After Blepharoplasty
